Billing Questions
Will my insurance pay for a Berkeley HeartLab test?
Many insurance companies and Medicare provide coverage, but it may depend on your insurance policy and medical necessity. Please call Customer Support at 1-800-432-7889, option 2 for more details.
Which commercial insurance carriers does Berkeley HeartLab contract with as an in-network laboratory?
A few carriers include United Healthcare® Insurance Company, MultiPlan®, and Beech Street®. For a full list of commercial insurance carriers with whom Berkeley HeartLab is contracted, please see the Insurance Providers section on this website.
How much does a Berkeley HeartLab test cost?
PPO patients generally pay between $50-$149. If you are covered by Medicare Part B, you will likely have no out-of-pocket cost. For self-pay and HMOs, the cost can vary. Call Customer Support at 1-800-432-7889, option 2 to determine your out-of-pocket cost for the tests your doctor ordered.
I received a check from my insurance company. What do I do now?
Please endorse the check from your insurance company to Berkeley HeartLab by writing “Pay to the Order of Berkeley HeartLab, Inc.” on the back of the check. Mail the endorsed check with a copy of your EOB (Explanation of Benefits) to us at the following address:

How much does the 4myheart Program cost?
Berkeley HeartLab testing fees include a charge of $10.00 for access to the Berkeley HeartLab 4myheart Cardiovascular Disease Risk Reduction Program.
Can I pay my bill now?
Berkeley HeartLab accepts VISA, MASTERCARD, AMERICAN EXPRESS, and DISCOVER credit card payments over the phone (1-800-432-7889, option 2). We also accept mailed payments in the form of checks. Make sure your account number is written on the check.
My insurance company is requesting a letter of Medical Necessity? How do I get this document?
Berkeley HeartLab, Inc. will take care of this. Please fax us (650-697-8704) a copy of your EOB (Explanation of Benefits) and we will respond to the insurance company’s request.
Do I need to pay my deductible as stated in the EOB?
Please call Customer Support at 1-800-432-7889, option 2 for pricing information or refer to your Berkeley HeartLab bill.
Will Medicare cover the cost of my test?
Berkeley HeartLab testing is reimbursed by the traditional Medicare Part B fee-for-service plan. Berkeley HeartLab, Inc. charges Medicare HMO (Medicare Advantage) patients the Medicare reimbursement rates. Please contact Customer Support at 1-800-432-7889, option 2 for more information about co-payments and pricing.
I need to know what my insurance company will pay. What are the procedure codes?
You may contact our Customer Support department at 1-800-432-7889, option 2 to obtain procedure codes, and a Customer Support specialist will explain approximately how much you will be paying out-of-pocket for the tests that your doctor is ordering or has ordered for you.
General Questions
How can I obtain my lab report?
You may obtain your lab report either from the health care provider who ordered the Berkeley test for you OR if your doctor has already reviewed those results with you and you have a patient ID number, you may access it through the 4myheart.com website.
How do I login to the 4myheart.com website?
Instructions for logging into 4myheart.com can be found on the 4myheart.com home page. If you cannot login, please call 1-800-432-7889, option 4 and ask for 4myheart Support.
How do I get Berkeley HeartLab tests?
Your doctor must order the tests and complete a requisition form. If your doctor is not familiar with the Berkeley tests, you may request information through Berkeley HeartLab Customer Support (1-800-432-7889, option 3).
If your doctor needs a test kit, they can be order it through Customer Support. Once your local blood draw site obtains your specimen, they will send it to Berkeley HeartLab via overnight delivery.
My doctor wants me to enroll in your 4myheart Program. What is it?
After your doctor reviews your results with you, he or she will develop a treatment plan for you and will probably refer you to the 4myheart Program. The Berkeley 4myheart Program is available to you as a part of your Berkeley Test for nine months. You will not be charged an additional fee. Please review the 4myheart.com website for more information.
At your convenience, a Berkeley HeartLab Clinical Educator is available to educate you about how to reduce your risk for cardiovascular disease and stroke. How long does it take for test results to be released?
Test results are usually released within 14 days of receipt of your specimen by Berkeley HeartLab.
